Personal “Hands On” Care
At Caring Hearts of Rochester (CHOR) we understand that finding experienced and compassionate caregivers can be challenging. While many of our clients still enjoy their independence, they may require some assistance with certain daily activities and tasks. Our qualified care professionals strive to go beyond the standard of care to reach higher levels for the individuals and families they serve.
As directed by you, or in conjunction with your medical providers, our Director of Patient Services (DPS) will create a personalized Plan of Care (POC) and oversee its delivery by our professional care staff which may be comprised of a Home Health Aide (HHA), a Personal Care Aide (PCA), a Licensed Practical Nurse (LPN) a Registered Nurse (RN) or any combination thereof.
Personal Care Services – Individual Care Plan
The Plan of Care (POC) will be derived of an assessment of your current needs, your recent and prior history, and your healthcare goals.
Our Director or Patient Services and our Nurse Supervisors will oversee your health conditions, medications, and treatment objectives. They have the ability to discuss health-related topics with your healthcare providers, and ability to recognize signs and symptoms to implement early interventions that may be critical for your health.
Additionally, our Nurse Supervisors will serve as liaisons at the time of discharge from the hospital or rehabilitation facility to ensure a smooth home transition (Pathways Home). Your medications and supplies can be set up prior to arriving home. Together with your physician and family members, we will participate in the care plan development for your non-medical needs, will provide ongoing care plan oversight and will ensure caregivers are trained to properly manage your healthcare issues.
To ensure we fully comprehend your individual needs, our Director of Patient Services or a Nurse Supervisor will conduct a comprehensive care evaluation. Together, with your input, we will create this Plan of Care (POC).
Your POC will include the initial nursing assessment and regular wellness visits designed to confirm that your care goals are being met or modified as needed. We will work with your physician, other care providers and your family as necessary to develop the plan and frequency of visits.
At no additional cost to you we will include a Home (or Environmental) safety assessment, a Family Planning Meeting, a Community resource review and offer Education and other Information.

- Sponge Bathing
- Grooming/Personal Hygiene
- Incontinence care/toileting
- Oral Hygiene
- Special Diet and Meal Preparation
- Positioning and ambulation
- Assisted recovery after hospital stays
- Meal preparation
- Homemaking services
- Grocery shopping and errands
- Transportation to appointments
- Medication set up and reminders
- Coordination of RN/doctor’s visits
- Respite care for family caregivers
- Supportive care for hospice patients
- Around-the-clock supervision (for individuals experiencing the effects of Alzheimer's Disease, Parkinson's Disease, dementia, stroke or are simply not able to be alone)

Grandma’s caregiver brings so much joy to her life. She enjoys the conversation and the time they spend together. I have been working with Caring Hearts of Rochester for three and a half years. I am so very grateful for the wonderful care they provide to my mom. I live out of state and used to worry about mom all day long. I no longer worry about mom, ever! I visit quite frequently and enjoy the conversation & companionship of the caregivers.
