Disease-Specific Care
Caring Hearts of Rochester believes that although many symptoms may present with like diseases, every person and every condition requires specific services tailored to the individual. A Plan of Care (POC) will be written based on input from the Caring Hearts of Rochester team, you - our client, your physician, and your family members involved in your care.
If it’s an acute illness or managing more than one condition, a specialized care approach is written and implemented to get the best results.
Our team members will work closely with your healthcare providers to develop a plan specific to you or your loved one’s needs. Our programs include but are not limited to specialized management of Heart Disease, pulmonary conditions, Diabetes, ALS, MS, Stroke, Alzheimer’s, Parkinson’s and other diseases in which dementia is manifest. Our programs involve preventative measures and treatment plans for both our medical and non-medical team under the coordination of the Registered Nurse and Director of Patient Services.
Caring Hearts of Rochester Nurses will monitor signs and symptoms, manage medication regimen, develop a healthy nutritional and hydration plan, and define tasks to which aides and caregivers will follow and report.
Caring Hearts of Rochester will evaluate home settings and will make recommendations for home safety improvement. We will adapt to and support Physical Therapy (PT), Occupational Therapy (OT), Speech and Language Therapy (SPL) and Social Work programs and goals, as outlined by your professional to accomplish short and long-term goals to maintain and improve the best quality of life. Our Aides and Caregivers will be trained to the proper techniques for ongoing home programs specific to needs.
Caring Hearts of Rochester Aides and Caregivers will follow the aides care plan to assist with activities of daily living (ADL’s) while focusing on disease-specific tasks of monitoring signs and symptoms, recording as necessary, ensuring proper positioning, encouraging exercises, preparing appropriate nutritious meals, documenting and reporting on changes in the conditions to the Nurse Care Managers.
CHOR Disease Management Programs
Caring Hearts of Rochester Heart
CHOR Heart program was developed to address care needs of our patients with Chronic Heart Failure (CHF) and heart disease.
Caring Hearts of Rochester Lungs
CHOR Lungs program was developed to help patients with Chronic Obstructive Pulmonary Disease (COPD) to improve quality of their daily life, to teach and assist with skills to manage chronic disease in the home environment.
Caring Hearts of Rochester Diabetes
Caring Hearts of Rochester Diabetes program was developed to address care needs for people with type I and II Diabetes. The Caring Hearts of Rochester nursing team is trained to provide Ongoing patient self-management education and support. This is critical to preventing acute complications and reducing the risk of long-term complications.
Caring Hearts of Rochester Joints
Caring Hearts of Rochester Joints Program was developed for patients who undergo knee and hip joint replacement. The program is focused on improving mobility, strength, and balance so that patients can get around and continue with outpatient therapy if needed.
Caring Hearts of Rochester Memory Care
Caring Hearts of Rochester Memory Care programs were established to provide care at home for patients with Dementia and Cognitive disorders including but not limited to Parkison’s and Alzheimer’s diseases.
Caring Hearts of Rochester has certified Dementia Care Specialists (DCS) trained by the Alzheimer's Association who will implement and provide specialized care in the home or facility setting to increase comfort and to encourage a successful day.
ALS and MS Support Programs
Caring Hearts of Rochester also offers staff training to provide support and management of patients with neurodegenerative diseases, such as ALS and MS.

Grandma’s caregiver brings so much joy to her life. She enjoys the conversation and the time they spend together. I have been working with Caring Hearts of Rochester for three and a half years. I am so very grateful for the wonderful care they provide to my mom. I live out of state and used to worry about mom all day long. I no longer worry about mom, ever! I visit quite frequently and enjoy the conversation & companionship of the caregivers. Mom’s caregivers see her daily and they help her with everything from cooking and cleaning to bathing and shopping. They are kind, compassionate, loving and have truly become family to mom and me. My mother doesn’t live nearby so I wanted someone to be with her a few days a week while I was at work. I have peace of mind knowing a responsible and kind caregiver is with her when I can’t be. Thank you very much for your wonderful care of mom.
